Responsibilities
Using the cognitive skills of application, analysis, synthesis, and evaluation, the Home Health Physical Therapist is responsible for using the concepts of case management to provide, coordinate and supervise physical therapy services of all patients: neonate, infant, child, adolescent, adult, and geriatric. Key functions include providing complex direct patient care, patient/family teaching, patient/family support and advocacy. The Home Health Physical Therapist provides direct physical therapy as prescribed by the attending physician under applicable state licensure status and in compliance with University Hospital agency policies and procedures. Accountability will be monitored through direct observations; achievement of service goals including satisfaction, quality, and financial outcomes; and performance improvement activities.
Qualifications
EducationGraduate of a four-year program in physical therapy approved by the American Physical Therapy Association and the Council of Medical Education of the American Medical Association.Licensure/CertificationCurrent and valid license for the practice of physical therapy in the state of Georgia or South Carolina, as appropriate. Current BCLS required at date of hire or before visiting patients on an independent basis.Valid driver's license and current automobile liability insurance required. Report of Motor Vehicle Record for past 5 years required.ExperienceNormal experience gained in physical therapy school required. At least one year physical therapy experience strongly preferred. Previous home health care experience is strongly recommended. Additional hospital or physical therapy experience in other settings is also a plus.Knowledge, Skills and AbilitiesAbility to think critically and prioritize multiple tasks and responsibilities.Physical therapy knowledge of growth and development relevant to the ages of the patients served.Physical therapy knowledge of the diseases and health care needs of the patient population served.Assessment, analytical, and synthesis skills sufficient to interpret and analyze patient's physical therapy data; prioritize needs; and develop an individualized plan of care.Evaluation skills adequate to evaluate patient response to care and take appropriate action.High level of interpersonal skills in order to effectively communicate with patients, families, physicians and other home health care personnel with whom daily interactions must take place.Basic knowledge and skills in management sufficient to effectively supervise other home health staff.Problem solving skills sufficient to allow effective problem/ conflict resolution.Ability to function with minimal supervision.Ability to provide reliable means of transportation.Demonstrated traits of initiative and dependability required.Ability to perform efficiently and effectively under stress.Philosophy of health care service is compatible with that of the mission of University Hospital.Ability to be flexible.Physical DemandsStanding for long periods of time (6-8 hrs.).Sitting for extended periods of time (at desk telephoning, making schedules, writing reports).Full range of motion including handling, lifting and turning patients, and transferring patients from chair to bed.Bending and stooping (as needed to provide physical therapy services, treatments and preventative procedures as required).Walking for long periods.Lifting of heavy patients and supplies (supplies weighing approximately 25 pounds).Sensory: sight to observe patient symptoms auditory to hear patients tactile to feel patient symptoms (warmth, induration) olfactory to smell disease specific smells

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
